If you have a deductible, please note you will be obligated to pay the full session rate until the deductible has been met.
Out of Network Insurance
All those who have insurance to assist with this fee are expected to handle the reimbursement for services rendered with their company themselves. We are willing to provide invoices of the services provided to assist you with this task. It is your responsibility to see that the fee is covered. If you will be filing with your insurance company, it is IMPORTANT that you realize we must assign a diagnosis, and that diagnosis will permanently be a part of your medical record. Payment is due at the time services are rendered.

Cancellation Policy
We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting a much needed session. Conversely, the situation may arise where another patient fails to cancel, and we are unable to schedule you for a visit due to a seemingly “full” appointment book. Perfect Balance Counseling requires that a credit card number be kept on file to reserve your scheduled services. If an appointment is not canceled at least 24 hours in advance you will be charged a full session fee.
